Personal Liability Waiver *	I have read and agree to the terms of the Personal Liability Waiver below.In consideration of Nazareth Retreat Center (NRC) and the Sisters of Charity of Nazareth permitting me to utilize the facilities, waters, buildings and grounds at NRC, I hereby, for myself and, heirs, executors, administrators, and/or assigns, waive and release any and all rights and claims of any nature that I and/or my family may have against NRC and the Sisters of Charity, it’s officers, employees, agents, assignees, licensees, their representatives, heirs, executors, administrators, successors, and/or assigns for and against any and all injuries or damages of any nature which I and/or my family may suffer while taking part in any activities at NRC. I certify that I am authorized to execute this release on behalf of myself. After use of the subject facilities has been completed, the Licensee agrees to leave the facilities and any and all equipment in a clean and orderly condition and in the same condition as they were prior to Licensee’s use, except for normal wear and tear. Individuals are liable for any property damage due to their activities. I have read and understand the guidelines and agree to abide by them. Lessee shall remain liable for any liability arising from Lessee’s use or occupancy of the Lessor’s premises or facilities; except injury or damages that may occur because of the negligence or willful and wanton misconduct of the Lessor, Lessor’s officers, agents or employees. Lessee will be responsible for any damage to premises caused by the Lessee’s activity while using the premises.
